Section 62: Assignment by endorsement or separate instrument.

Assignment of a policy of life insurance may be made by endorsement on the policy or by a separate instrument in the following words to that effect namely "[------------ ] of ------ in consideration of ------ do hereby assign unto ------, his executors, administrators, and assignees, the (within mentioned) policy of life insurance granted ------- of ------
(here describe the policy).
In witness whereof I have hereunto set my hand and seal this ----- day of ------, 20--- ".
